Intratympanic gentamicin treatment after endolymphatic sac surgery.

Interval treatment with up to three intratympanic gentamicin injections once weekly effectively controlled vertigo while preserving hearing in patients with Ménière's disease and recurrent or resistant vertigo after saccotomy.Recurrent or resistant incapacitating vertigo may occur after endolymphatic sac surgery (saccotomy) in patients with Ménière's disease. In these patients, revision saccotomy, vestibular nerve section or labyrinthectomy are the established treatment options. Who is at risk for ongoing dizziness and psychological strain after a vestibular disorder?

Patients with vestibular vertigo syndromes often suffer from anxiety and depression, whereas patients with psychiatric disorders often experience subjective unsteadiness, dizziness, or vertigo. Thus, it has been hypothesized that the vestibular system may be interlinked with the emotion processing systems. The aim of the current study was to evaluate this hypothesis by correlating vestibular and psychiatric symptoms with the course of the disease over 1 year. Intratympanale Intervalltherapie des Morbus Meni�re mit Gentamicin unter Erhalt der Kochleafunktion

Patienten und Methode Zwischen 1997 und 2000 haben wir 57 Patienten intratympanal einseitig mit Gentamicininjektionen (jeweils 0,3 ml = 12 mg) therapiert. Eine Therapieserie bestand maximal aus je einer Injektion an Tag 1, 8 und 15. Intratympanal injiziertes Gentamicin akkumuliert langsam (Durchschnitt unseres Krankenguts: 4,6 Tage) im Innenohr. Die Intervalltherapie vermeidet uberhohte Gentamicinkonzentrationen. Sobald Zeichen einer Innenohrreaktion auftreten (Nystagmus, Unsicherheit, Schwindel), hat die Therapie gewirkt und darf keinesfalls weitergefuhrt werden.

Treatment of Ménière's disease by low-dosage intratympanic gentamicin application: effect on otolith function.

Objectives: The intratympanic application of a low dosage of gentamicin is increasingly favored as treatment for Meniere's disease. While posttreatment observations have confirmed a long-term success of the therapy of vertigo attacks, clear differences in the posttreatment recovery interval can be observed. In addition to differences in central-vestibular compensation, the degree of peripheral vestibular damage, i.e., to the saccule, utricle, and semicircular canal ampullae, varies among patients. When tinnitus loudness and annoyance are discrepant: audiological characteristics and psychological profile.

This study evaluates sociodemographic and clinical characteristics of patients reporting discrepant levels of tinnitus loudness and annoyance. 4958 subjects recruited from a national tinnitus association completed a comprehensive screening questionnaire including Klockhoff and Lindblom’s loudness grading system and the psychometric Mini-TQ (Tinnitus Questionnaire). There was a moderate correlation of 0.45 between loudness and annoyance. Of the subjects reporting very loud tinnitus, about one third had only mild or moderate annoyance scores. Long-Term Results after Interval Therapy with Intratympanic Gentamicin for Meni??re???s Disease

Objectives The new single-shot and interval treatment for Meniere's disease with gentamicin was designed to avoid cochlear damage during treatment with gentamicin. Methods To date, 90 patients were treated with the single-shot or interval gentamicin therapy. Fifty-seven cases of Meniere's disease were followed up prospectively between 2 and 4 years. Evidence of unilateral isolated utricular hypofunction

The findings demonstrate that an enduring unilateral utricular dysfunction, possibly together with canal hypofunction, can occur after labyrinthine disease or injury. They also suggest that unilateral, isolated utricular dysfunction - or utricle paresis - can occur, representing a novel entity in the differential diagnosis of peripheral vestibular function.
